Social worker EPA

• You are the point of contact for residents with serious mental health issues and assist them in finding appropriate help and solutions. This includes topics such as independent living, managing money, loneliness, work and how someone spends their day.
• From the very first meeting, you will be working together with mental health colleagues: you will conduct the intake together, draw up the support plan together and implement it. To this end, you will work together with various organisations, such as welfare institutions and the Municipality of Leeuwarden, to ensure that residents receive the right help. You will become part of the FACT Centre regional team.
• You are the point of contact for residents and actively involve their network, focusing on their own strengths and the strengths of their environment.
• You connect requests for assistance with residents' initiatives, volunteers, neighbourhood facilities, and other stakeholders.
• You will help shape this constantly changing role.

• A higher professional education (HBO) bachelor's degree, HBO working and thinking level, and knowledge of legislation and regulations in the social domain.
• You are confident, stress-resistant and flexible.
• Experience and affinity with the EPA target group.
• Sensitive in contact and able to switch easily between different environments and disciplines.
• Digitally literate, independent, and creative.
• Proactive and able to motivate colleagues and partners.
• I would like to work in a dynamic environment, where there is room to help build an approach that is still under development.
• Possesses a Class B driving licence (own transport is desirable, given our wide working area).

• A competitive salary in accordance with the Social Work collective labour agreement, ranging from a minimum of €3,651 to a maximum of €5,769 gross per month (scale 9, based on 36 hours per week), supplemented by excellent terms of employment such as an Individual Choice Budget and a career budget.
