Supervisor
Will you be our new colleague in Social Neighbourhood Team South? We are looking for a supervisor for 28-32 hours.
You:
- implements social guidance for residents;
- Manage your own caseload, make suggestions for meaningful weekly activities and engage in dialogue about them. All this to build a network and be meaningful;
- encourages the resident to take steps from their own strengths towards as much independence and autonomy as possible and looks for new opportunities to connect residents to existing activities;
- Visits residents in their homes;
- Identifies and reports the resident's situation;
- thinks about the content of support plans and how it will be implemented in practice;
- identifies needs, opportunities, bottlenecks and problem situations in the resident that are insufficiently addressed in the support plan or could be complementary to it;
- works on the basis of the action plan drawn up by the social worker in the social neighbourhood team and makes proposals on practical implementation that lead to improvements in support;
- Liaises and collaborates with other healthcare providers and agencies;
- Guides the interaction between parent and child;
- stimulates, motivates and activates to engage in activities that increase self-direction and self-reliance;
- Scales up to a social worker where necessary;
- Have a completed senior secondary vocational education at least at MBO level 4 focusing on human, social and behavioural issues;
- Have experience and background knowledge of the target group's issues;
- Have knowledge of finance and administration so that appropriate support can be provided;
- You have empathy, can coach well, are customer-oriented and flexible;
- can collaborate and connect and you are a strong personality;
- keeps residents informed about the progress of works.
